Nominate an Outstanding Caregiver Today!

The 2024 Outstanding Caregiver Awards will be presented at the 20th annual community Caregiver Recognition Mass (formerly White Mass) on Sunday, November 17, 2024 at 11:00 am at the Cathedral Basilica St. Peter in Chains in downtown Cincinnati.
Use this form to submit one nomination for either an EMPLOYEE/PROFESSIONAL or for a VOLUNTEER/FAMILY MEMBER.
Please double check your nomination for the correct information using home addresses for the nominees so we can send them a formal invitation to the event. Invitations to the event will be sent in early October. Feel free to send your own "Save the Date" note congratulating your nominee(s).
